South Florida AIDS Network (SFAN), part of the Jackson Health System, provides

• Case Management for HIV+ persons,
  • which begins with an assessment of the clients medical
  • financial,
  • educational,
  • psychosocial and referral needs.

An individualized plan is developed, which can include

• advocacy,
• linkages and referrals to needed medical care,
• dental care,
• home health care,
• HOPWA,
• ADAP,
• PAC Medicaid Waiver,
• mental health,
• substance abuse, and/or bereavement counseling,
• peer support,
• educational groups,
• prescription assistance,
• food vouchers,
• housing and utility assistance,
• temporary shelter placement and transportation assistance.

Educational services are available to the general public.

The agency has three locations, and offers free, confidential HIV testing at its main office only.
