Description
Aids help, inc provides project aids care client eligibility.
General requirements:
- Medicaid eligible (SSI, AFDC),
- AIDS or HIV Diagnosis,
- patient meets level of care criteria,
- waiver services cannot exceed cost of institutional care.
Special Income Level:
- applies to hospital and nursing home groups, income under $1158/month. R
- esources under $2000, must be disabled.
Procedures:
1) Case manager completes client screen;
2) Client referred for eligibility determined;
3) State determines disability and financial eligibility;
4) State determines level of care;
5) State notifies CM of eligibility;
6) Case Manager assesses need/designs care plan;
7) Case Manager notifies client and DCF of final eligibility;
8) State notifies client of enrollment;
9) Case Manager authorizes service delivery;
10) Case Manager bills Medicaid;
11) Service providers send claims to Case Manager;
12) Case Manager authorizes claim for payment;
13) Medicaid pays claim;
14) Plan of care eligibility, level of care reassessed annually.
