Journey of hope
Provides financial support of up to a
maximum of $500 for basic needs for cancer patients and their families
(not all applicants are approved, and if they are, there is no guarantee
of receiving maximum funding).
Will only pay for items and services that
could be considered a critical survival need, such as: rent and security
deposits, mortgage payments, essential utility bills, hospital expenses,
automobile payments and automobile insurance.
Does not cover co-payments
or reimburse for bills that have already been paid.
Will not pay for
cable TV, cell phones, print subscriptions, credit cards, auto repairs,
moving expenses, food, toiletries, or clothing.
This service is provided
for cancer patients who have extinguished all other avenues for aid.
No
money is given directly to the client.
Agency may provide funds jointly
with other agencies.HOW SERVICE IS PROVIDED: By mail; By phoneWHO
QUALIFIES: Cancer patients 18 and older who reside in Cuyahoga or Lorain
Counties (must have a permanent address).
Must be receiving active
cancer treatment within one year of diagnosis date.
Applicants who
receive assistance may be approved for another grant after one year if
in active treatment, but this is not guaranteedHOURS OF SERVICE: M-F:
9:00am-5:00pmFEES: FreeREQUIRED DOCUMENTS: Completed application must be
signed by patient s oncologist and a licensed social worker.
Will also
need copies of bills to be considered for payment; proof of need such as
copy of mortgage payment, letter from landlord, utility shut-off notice
etc.HOW TO ACCESS: Must have referral from an Oncologist or licensed
social worker.
Applications can only be submitted through a medical
social worker (if the social worker does not already have an
application, they may phone to request one M-F: 9:00am-5:00pm).
Application must be submitted from the 1st through the 15th of the
month.
Late or incomplete applications will be returned and may be
resubmitted the next month.
Social workers will be informed on the
status of their patient s application within 30 days upon receipt of
application.
May phone M-F: 9:00am-5:00pm for information, however,
information on the status of an application will only be given to the

*Homelessness Prevention and Rapid Re-Housing Program*
On February 17, 2009, President Obama signed the American Recovery and
Reinvestment Act of 2009,which ludes $1.5 billion for a Homelessness
Prevention Fund. Funding for this program, called the Homelessness
Prevention and Rapid Re-Housing Program (HPRP), is being distributed
based on the formula used for the Emergency Shelter Grants (ESG) program.
Grantees may provide rental assistance or services to eligible program
participants who are residing or would like to reside outside their
jurisdiction. They may also fund ubgrantees that serve persons outside
of the jurisdiction, subgrantees that are located outside of the
jurisdiction, and subgrantees that serve multiple jurisdictions. A
grantee may establish more stringent requirements, such as requiring
that HPRP funds be spent within the jurisdiction or establishing a
residency requirement,but HUD is not setting these requirements.
