American Kidney Fund
The American Kidney Fund (AKF) is
the nation's leading non-governmental source of direct financial
assistance for needy kidney patients.
AKF offers to those who qualify
the following services: direct patient grants when other sources of
assistance have been exhausted to pay for treatment related expenses
(medication, transportation and special diets).
Free pharmacy products:
Nephro-Vite +FE, a renal vitamin-plus; SangCya, an immunosuppressive
drug used to prevent organ rejection; and Hectorol.
Discount Pharmacy
Card program which provides savings up to 30% through PRxN Pharmacy
Network, a mail service pharmacy.
A Health Insurance Premium Program
(HIPP) to help low-income dialysis patients maintain their health care
coverage.
Education programs for the general public to raise awareness
and prevent kidney disease.
And a national toll-free help line which
provides information about kidney disease, its prevention and treatment,
and organ donation.
The Renagel Patient Assistant Program is made
possible through Genzyme Corp.
and is intended for individuals who have
have no means of affording it, directly or indirectly, through insurance
coverage or federal, state or local programs. To qualify for assistance,
patients must be referred to the American Kidney Fund by their

American Kidney Fund Financial Assistance
SERVICE DESCRIPTION: Financial
Assistance for People Living with Kidney DiseaseIndividual Grants
Program- provides financial assistance to qualified dialysis patients
who are referred by their physicians and social workers.
-Grants are
provided for treatment-specific expenses such as transportation,
over-the-counter medicines, medication co-payments, kidney donor
expenses and other necessities such as dentures.
Pharmacy Grant Program-
The Pharmacy Grant Program enables qualified renal patients to receive
urgently needed medications, nutritional products, and durable medical
supplies.Disaster Relief Program- Disaster Relief Program helps ESRD
patients get back on their feet when catastrophic events strike their
communities. -
This program provides funds to assist patients replace
medications, food and household items and pay for other necessities lost
because of natural disasters such as flooding, hurricanes, etc.Sherer
Travel-Related Dialysis Program- assists eligible dialysis patients to
afford treatment while traveling.
Patients or facilities may be
reimbursed for the 20% of treatment costs not covered by Medicare or any
other source for emergency transient dialysis.Health Insurance Premium
Program-Through HIPP, AKF pays Part B Medicare, Medigap and Cobra
premiums for dialysis patients who have insufficient income and savings,
allowing them to continue their coverage.Rx Assistance-The Hectorol
(doxercalciferol) Patient Assistance Program provides 2.5 mcg, 0.5 mcg
capsules and ampules.
The Renagel (sevelamer hydrochloride) Patient
Assistance Program provides Renagel 800 mg tablets.
Both programs offer
up to a four-month supply to eligible patients.
If necessary, patients

*Homelessness Prevention and Rapid Re-Housing Program*
On February 17, 2009, President Obama signed the American Recovery and
Reinvestment Act of 2009,which ludes $1.5 billion for a Homelessness
Prevention Fund. Funding for this program, called the Homelessness
Prevention and Rapid Re-Housing Program (HPRP), is being distributed
based on the formula used for the Emergency Shelter Grants (ESG) program.
Grantees may provide rental assistance or services to eligible program
participants who are residing or would like to reside outside their
jurisdiction. They may also fund ubgrantees that serve persons outside
of the jurisdiction, subgrantees that are located outside of the
jurisdiction, and subgrantees that serve multiple jurisdictions. A
grantee may establish more stringent requirements, such as requiring
that HPRP funds be spent within the jurisdiction or establishing a
residency requirement,but HUD is not setting these requirements.
